PRE-EMPLOYMENT DRUG TESTING CONSENT AGREEMENT
As a prerequisite to employment, I hereby consent to a drug and alcohol test and to furnish a sample of my urine and/or saliva for analysis, as shall be determined by Southern Mechanical Contractors in order to meet with their policy regarding the selection of applicants for employment. I understand that I have the right to request a re-test of the initial specimen at a licensed laboratory of my choice when I have a positive test for drugs. All requests for re-test of the sample must be paid for by the applicant and requested within twenty-four (24) hours after being disqualified. I further authorize and give full permission to have the Company and/or its authorized agents to send the specimen or specimens so collected to a laboratory for a screening test for the presence of any prohibited substances under the policy, and for the laboratory or other testing facility to release any and all documentation relating to such test to the Company and/or to any governmental entity involved in a legal proceeding or investigation connected with the test. I also authorize the Company to disclose any documentation relating to such test to any governmental entity involved in a legal proceeding or investigation connected with the test.
I understand that the results of the drug and alcohol testing of my urine and/or saliva, if confirmed positive will remove me from consideration for employment. I also understand that if I refuse to consent, I will be removed from further consideration of employment.
I understand that only duly-authorized Company officers, employees and agents will have access to information furnished or obtained in connection with the test; that they will maintain and protect the
confidentiality of such information to the greatest extent possible; and that they will share such information only to the extent necessary to make employment decisions and to respond to inquiries or
notices from government entities.
I will hold harmless the Company, its agents and any testing laboratory the Company might use, meaning that I will not sue or hold responsible such parties for any alleged harm to me that might result from such testing, including loss of employment or any other kind of adverse job action that might arise as a result of the drug or alcohol test, even if a Company or laboratory representative makes an error in the administration or analysis of the test or the reporting of the results. I will further hold harmless the Company, its agents and any testing laboratory the Company might use for any alleged harm to me that might result from the release or use of information or documentation relating to the drug or alcohol test, as long as the release or use of the information is within the scope of this policy and the procedures as explained in the paragraph above.
I understand that it is the current use of illegal drugs that would prohibit me from being employed by this company.
Further, I understand that if employed by the Company, I must abide by the terms of the Company's drug-free work place policy and may be required to submit to testing for the presence of illegal drugs or alcohol. I understand that submission to such testing is a condition of employment with the Company, and disciplinary action, up to and including discharge, may result if (1) I refuse to consent to testing; (2) I refuse to execute all forms of consent and releases of liability as are usually and reasonably attendant to such examination; or (3) I otherwise violate the policy.
I further agree that a reproduced copy of this pre-employment consent and release form shall have the same force and effect as the original.
This policy and authorization have been explained to me in a language I understand, and I have been told that if I have any questions about the test or the policy, they will be answered. I have carefully read and understand its contents. I acknowledge that I have not been coerced into signing this document by anyone.

JOB SPECIFICATIONS
GENERAL REQUIREMENTS: For each position listed below you must have the ability to climb ladders, frequently lift, push, or pull heavy materials ranging from 5-150 lbs., bend, squat, work from scaffolding (and other OSHA approved working surfaces), have knowledge of blueprint reading and the ability to work with minimum supervision. Each employee will need to provide certain tools required for the trade.
PIPE FITTER/HELPER
- Ability to fit all screwed and welded pipe
- Ability to be equipped with and use required hand tools
- Ability to use measuring devices related to doing pipe work
PIPE WELDER:
- Ability to do above Pipefitter/Helper requirements
- Ability to fit and weld all ferrous and non-ferrous metals using machines available
SHEET METAL MECHANIC/HELPER
- (Shop) Ability to measure, shear and form metal
- (Field) Ability to assemble and install ductwork
- Ability to be equipped with and use required hand tools
- Ability to use measuring devices related to doing sheet metal work
TRUCK DRIVER
- Must have a valid GA driver's license
- Must not have more than two points on license at any given time
